Angel Card Reading Request Form (8 card reading - cost £12.50 UK) * If you are going to pay with a friend or relative's card or Paypal, please enter their surname in brackets in the "NAME" box next to yours, if it differs from your own. Your Full Name: Your Date of Birth: Country of Residence: Your E-Mail Address: Submitting your request will take you to the "THANK YOU" & "BUY NOW" page from where you can pay by either using your Debit/Credit card or directly through your Paypal
